Clinical Experiences of Pulmonary Artery Catheterization for Cardiac Anesthesia in



Abstract
Pulmonary artery catheterization has been a useful but invasive monitoring method to assess pump function, intravascular volume, vascular resistance, etc. for certain indicated patients.
Due to its invasive nature, anesthesiologists imperatively need information about methodology and complications of pulmonary artery catheterization for accurate data interpretation.
To accumulate clinical experiences, authors analyzed 50 cases of pulmonary artery catheterization in adult open heart surgery. For convenience. we divided 50 cases of patients into 2 groups. Group I (n=22) included CABG surgery, group II (m=28)
included
valvular surgery and aortic aneurysm respectively.
The results are follows :
1) In most cases (m=44, 88%), right internal jugular vein was selected for pulmonary artery catheterization.
2) complications during pulmonary artery catheterization were arrhythmia (n=7). Carotid perforation (m=5) and failure to obtain PCWP(n=2) in order.
3) Distance from skin for obtain PCWP was 54¡¾3.8§¯ for male, 52¡¾3.9§¯ for fomale in group I and 56¡¾5.2§¯ for male, 51¡¾3.8§¯ for female in group II respectively.
4) Intrathoracic distributions of catheter tip were right inferior in most cases (n=45.90%)
5) Cardiac index after bypass was significantly increased than before bypass in both group, but changes of pulmonary artery pressure and pulmonary capillary wedge pressure during bypass were not significant in both groups.
